When you provide your mailing address, you’ll receive this CAD Care Kit and CAD Conversation Cards.*

You’ll be sent a CAD Care Kit in the mail. The CAD Care Kit includes a CAD fact sheet, cards with instructions for lab technicians, tools to prepare you for conversations with your healthcare providers, and a CAD Unraveled journal with a pen to help you keep track of questions for your care team.

A few weeks after you’ve received the CAD Care Kit, you will receive a deck of CAD Conversation Cards in the mail. These are designed to help you have conversations with your friends, family and healthcare team.

Expect to receive your Care Kit in 6 to 8 weeks.
